PAINT & SIP - REGISTRATION FORM
Please fill out this Form for EACH guest who will be attending.
STEP 1: Fill in the Guest Name, Email, any comments or questions you may have and click yes or no if you are 21 years old or not.
STEP 2: Add a 1 to the Quantity box under the Ready, Sip, Paint logo and click Add to Cart. A little black cart will appear in the upper right hand corner of this page. Click on it.
STEP 3: Click Checkout. Fill out the payment and contact information and then hit submit.
You should receive and email confirmation of your registration. If you do not, please email somerset.wi.chamber@gmail.com and request your confirmation be emailed to you.
